The following forms and tools are provided to assist with successful management of your Cycling Without Age (CWA) program. You may use them as templates for the content and adapt as needed to remove/replace Bike Fed references and logos.
Pilot Training
The following checklist is provided as a guide at the beginning of volunteer pilot training and is also used as an assessment tool during the certification process. It includes all the information that volunteers will need to be familiar with to become a successful pilot.
- Pilot Training/Test Checklist
- Pilot Procedures (customizable template)
- Pilot Policies and Agreement
- Acknowledgement and Receipt of Pilot Handbook Form
Passenger Waiver
A passenger waiver is required by most insurance companies, signed by the passengers or their decision-maker, to protect the organization and the trained volunteer pilots, in the event something unlikely or unexpected occurs.

Handlebar Info Tags
When pilots are pedaling passengers around the community, they may receive excited inquiries by curious passers-by. To share information without taking away from the passenger’s experience, we recommend creating a supply of small customized quick-reference sheets to share. A sample from a CWA program is provided below:
