Cycling Without Age: Library of Forms

Bike Fed offers you the following forms and tools to assist with successful management of your Cycling Without Age (CWA) program.

Pilot Training

The following checklist is provided as a guide at the beginning of volunteer pilot training and is also used as an assessment tool during the certification process. It includes all the information that volunteers will need to be familiar with to become a successful pilot.

Passenger Waiver

A passenger waiver is required by most insurance companies, signed by the passengers or their decision-maker, to protect the organization and the trained volunteer pilots, in the event something unlikely or unexpected occurs.

Maintenance

It is very important to maintain trishaws in good working condition to ensure safety for passengers and pilots. Well-maintained trishaws help reduce the number of costly repairs.

Handlebar Info Tags

When pilots are pedaling passengers around the community, they may receive excited inquiries by curious passers-by. To share information without taking away from the passenger’s experience, we recommend creating a supply of small customized quick-reference sheets to share. A sample from a CWA program is provided below:

For Current CWA Programs, Trainer/Educators, and Pilots:

Wisconsin is known for having more Cycling Without Age (CWA) programs than any other state in the country. If you have had anything to do with that over the years – Thank You! Bike Fed is honored to partner with communities, organizations, and skilled nursing homes around the state, but we don’t have a comprehensive picture of all the great programs, trainer/educators, and current pilots statewide.

We’d like to create connections, share resources and information, and demonstrate how the program is growing. We need your help! If you are already actively engaged with CWA, we would appreciate your time to complete the sections of the following CWA information form that match your role(s). It should take about 5-10 minutes depending on how many roles you have. And, please, share with others you know.

CWA Information Form

Thanks for reviewing this information on “Forms.” The next section includes a “Map of Skilled Nursing Facilities (SNF).”

SNFs are eligible for enhanced assistance through the Civil Money Penalty Reinvestment Program.
